Compose query to create a simple table using a statement

Implements a 'DBI' compliant interface to Presto. Presto is
an open source distributed SQL query engine for running interactive
analytic queries against data sources of all sizes ranging from

<li>a call to <code><a href="https://dbi.r-dbi.org/reference/Id.html">DBI::Id()</a></code> with components to the fully qualified table name,

<li>a call to <code><a href="https://dbi.r-dbi.org/reference/SQL.html">DBI::SQL()</a></code> with the quoted and fully qualified table name
